Responsibilities:
Provide direct patient care to residents, including assistance with activities of daily living (ADLs), medication administration, and vital sign monitoring
Develop and maintain positive relationships with residents, families, and interdisciplinary team members
Participate in resident assessment, care planning, and documentation
Collaborate with other healthcare professionals to ensure seamless coordination of care
Maintain accurate and timely records of patient care
Perform restorative care activities, such as ambulation, range of motion exercises, and toileting assistance
Adhere to Spring Creek Healthcare Center's policies and procedures, as well as applicable state and federal regulations
Requirements:
Current CNA certification in the state of New Jersey
High school diploma or equivalent required
