Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts and other assistive devices.
Monitor and observe patients’ health status by taking vital signs, recording medical information accurately in Electronic Health Record (EHR) systems, and reporting changes promptly.
Administer medications following prescribed protocols while maintaining compliance with HIPAA regulations to ensure confidentiality.
Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques and patience.
Support clients with medical procedures such as catheter care, wound management, and hospice-related needs under the supervision of healthcare professionals.
Assist with household tasks including meal preparation, cleaning, laundry, and maintaining a safe environment conducive to health and well-being.
Document all patient interactions meticulously in medical records or electronic documentation systems to ensure continuity of care.
Collaborate effectively with nursing staff, therapists, and family members to develop and implement personalized care plans tailored to each individual’s needs.
Requirements
Valid certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA), with current CPR certification.
Proven experience working in home health settings, assisted living facilities, nursing homes or long-term care environments.
Strong knowledge of medical terminology, medication administration protocols, and infection control practices including HIPAA compliance.
Demonstrated ability to work with individuals with disabilities or developmental disabilities; experience with Alzheimer’s care, dementia care, hospice care or memory care is highly valued.
Proficiency in using EMR/EHR systems for documentation and patient record management.
Ability to perform physically demanding tasks such as lifting patients with Hoyer lifts or assisting with mobility safely.

Salary
